The purpose of this form is to request that the Conviction Integrity Unit of the Travis County District Attorney’s Office review a conviction to determine if it is a case of wrongful conviction or actual innocence.

BY SIGNING BELOW, YOU UNDERSTAND THAT:
- No information that you give us is protected by attorney-client privilege. The Conviction Integrity Unit (CIU) does not represent you.
- The CIU is a part of the Travis County District Attorney’s Office. Our interest is in seeing that justice is done. By submitting this form, you are asking the CIU to investigate your claim of actual innocence or wrongful conviction.
- The CIU cannot process this application without the agreement of the convicted person (defendant).
- We recommend that you seek the advice of a lawyer regarding this process. If you are represented by an attorney, the CIU MUST communicate with your lawyer regarding the case.
- The CIU looks at every claim that we receive, however we do not perform a full investigation of every claim. Because of the volume of cases under review, the CIU cannot promise that we will be able to process your claim quickly.
